About prostate and possible treatment

How do I know that I suffer from prostatic problems?
Prostatic problems in men naturally occur with increasing age.

The first signs of problems with your prostate (apart from acute inflammation) have typically very slow onset and are not too annoying, so men often do not pay attention or attribute them to their increasing age.

The initial signals when you should start being more alert include:
  • weakening of your urine flow
  • intermittent flow of urine
  • difficult urination - e.g. need to push to begin urination.

Additionally, the following problems may occur:
  • more frequent urination than in the past
  • annoying urge to urinate several times per night
  • the feeling of full bladder even after you have finished urinating
  • dribbling at the end of urination, or into your underwear after you finish urination
  • incontinence (involuntary leakage of urine) in serious conditions.

It is interesting that the size of the prostate does not always directly relate to the severity of the symptoms that a man experiences. Subjective description of symptoms by a patient is therefore an integral part of the medical prostate examination.
What does BPH stand for?
Benign prostatic hyperplasia (also known as benign prostate hypertrophy) is a common prostatic problem usually affecting men at the age of 50+. Growing prostate compresses the urethra at its outlet from the bladder. Therefore, BPH can negatively affect urination - weakening of the urine flow, frequent urge to urinate, etc.

When should men start regular preventive checks?
The best age to start regular preventive checks is at the age of 50 once every 1-2 years. In case of positive cancer family history, the recommended age is 40.
When should I see the doctor?
In case of lower abdomen pain, pain during urination or if there is blood in your urine, see the doctor without delay. If you have problematic symptoms during urination, or if these symptoms worsen, we also recommend medical examination.
What does the prostate examination look like?
Standard examination consists of the overall evaluation of the subjective symptoms, blood and urine tests, and a gloved finger prostate examination through the back passage.
What is PSA?
Prostate-specific antigen (PSA) is a protein that is produced only by cells of the prostate gland. It is contained in the prostatic fluid, and small proportion is released into the bloodstream. PSA level can therefore be easily detected in blood tests.

PSA tests are used as one of the methods of general evaluation of changes in the prostate (enlargement of the prostate gland, inflammation, etc.). But the PSA value is also dependent on age, the size of the prostate and the overall man´s health. Therefore, to determine the exact diagnosis, the doctor performs additional examinations.
Can change of lifestyle help with enlarged prostate?
Minor changes in lifestyle can reduce some of the symptoms. For example:
  • If you reduce your fluid intake in the evening, you can reduce the frequency of night-time urination.
  • By limiting alcohol or coffee consumption you avoid irritating your bladder, which provokes the urge to urinate, even if the bladder is not yet full.
  • Urinating sitting down can facilitate better emptying of the bladder.
  • Sedentary way of life has a negative effect on the prostate, so regular exercise (walking, ...) is recommended during the week.
What treatment will the doctor recommend?
The doctor will recommend appropriate treatment based on your examination and diagnosis. The choice of the treatment also depends on the "severity" of the symptoms.

In the first stage of BPH (with mild urinary problems) the doctor usually recommends only so called "watchful waiting" - careful monitoring of your condition along with change of your lifestyle. Since the first symptoms appear, it is beneficial to take supplements which support the proper function of the prostate and urinary tract.

If symptoms worsen or in more serious cases of hypertrophied prostate, the doctor will start the treatment. The most commonly recommended treatment depending on the degree of the symptoms is currently:
  • herbal preparations with Saw Palmetto
  • alpha-blockers
  • 5-alpha-reductase inhibitors.

Only in cases of serious complications, the doctor will recommend one of the surgical options.
